The “Care” Face Sheet screen maintains information about each resident’s special needs and capabilities – cognitive and mobility issues, diagnoses, therapies, and special instructions.
Tracking Cognitive levels can help you design a Person Appropriate activities program, making sure that you’re providing the right balance of activities for your current resident population.
Knowing your current population’s mix of Mobility levels can help your facility staff know which residents will need assistance getting to your activities.

Diagnoses and Special Instructions, along with Cognitive and Mobility levels, are automatically printed on the Resident Outing Cards that you can distribute to staff or volunteers when they’re helping you with a resident trip outside your facility.
